Purpose

Under the direction of the Resettlement Supervisor, the Senior Case Manager will work with newly and previously arrived Afghan individuals and families to provide comprehensive case management and support services. The role focuses on helping clients achieve stability and self-sufficiency by addressing needs related to housing, health, education, and community integration.

 

 

Responsibilities

  • Oversee comprehensive service coordination for newly arrived and previously arrived Afghan clients and families.
  • Handle complex and high-need cases, including those requiring crisis intervention and intensive support.
  • Conduct thorough client assessments to identify needs, strengths, and barriers to self-sufficiency.
  • Develop individualized service plans and coordinate referrals with in-house programs and external partners to ensure holistic support.
  • Continuously monitor client progress, evaluate the effectiveness of interventions, and adjust support plans as needed.
  • Maintain accurate and up-to-date case records, including documentation of all client contacts, services provided, and outcomes.
  • Record clients’ public assistance usage and ensure completion of all required reports in compliance with program and funding guidelines.
  • Provide culturally responsive guidance, crisis intervention, and emotional support to clients navigating complex systems.
  • Stay informed about changing immigration policies, community resources, and best practices to improve service delivery.
  • Mentor and provide guidance to case management staff
  • Promote a collaborative and supportive team environment that prioritizes compassion, accountability, and service quality.
  • Build and maintain strong partnerships with local service providers, schools, and community organizations to enhance program impact and client outcomes.
  • Participate in community meetings, training, and capacity-building initiatives to strengthen organizational presence and collaboration.

Qualifications

Education and Experience:

  • Bachelor’s degree or equivalent work experience in social work, education, psychology, sociology, or other behavioral science in lieu of degree.
  • Minimum of 4 years’ experience in case management, including working with children and youth, previous work experience and interest working with foreign born populations is required.

 

Skills:

  • Must have superior written and verbal communication skills. Language fluency in written and spoken English in addition to refugee language is required.
  • Knowledge and experience in working with Microsoft Word, Excel, Power Point and database software is essential.
  • Strong professional ethics and sensitivity in dealing with confidential matters.
  • Excellent interpersonal skills, the ability to work independently and as part of a in a multi-cultural team.
  • Possession of a valid California driver’s license and access to a reliable vehicle.

Special Requirements

  • The successful candidate must pass a background check, reference checks and motor vehicle checks.
