Purpose

The Intensive Case Manager will provide collaborative mental health focused services to individuals and families to support the safe and timely case management. In addition, the Case Manager will provide intensive case management and clinically informed interventions using strengths-based and culturally and linguistically appropriate case management. These individuals have been identified as requiring a high level of care. The goal of the intensive interventions is to assist in stabilizing the individuals through close communication and collaboration with the client and additional case management team.  The Intensive Case Manager will conduct outreach and education to community service providers to supplement existing services that do not adequately address the unique needs of clients. The Intensive Case Manager will regularly interact with case managers and behavioral healthcare providers.  

Responsibilities

Essential Duties as Intensive Case Manager

  • Manages a caseload of cases in acute crisis and non-acute cases for mental health support.
  • Conducts consults with clients to promote safety and stabilization and assess risk and protective factors.
  • Regularly provides crisis intervention services to individuals and families, including safety planning and coordination with stakeholders.
  • Applies TF-CBT, MHFA, MI and other clinically informed approaches to address acute crisis.
  • Conducts client centered needs assessments.
  • Identifies viable resources, often in unfamiliar communities, to address reasons for referral and ensure access to basic supports (medical and mental health care focused). Assists individuals and families in addressing barriers to accessing needed resources.
  • Develops individualized SMART goals.
  • Completes documentation (case notes, goals, reports, assessment tools) in a timely manner and without prompting.
  • Coordinates internally with other supervisors and case workers to accept referrals of individuals as they transition in and out of acute crisis and releveling of service provision.
  • Provides psychoeducation to clients on relevant topics.
  • Supports individuals and families to cope with stressors.
  • Monitors progress of goal completion and adjustment steps for clients.
  • Monitors the safety and well-being of individuals including but not limited to indications of trafficking, abuse, and neglect. Creates and revises safety plans as needed.
  • Serves as a mandated reporter and works with state child welfare as needed.
  • Networks and develop partnerships within to educate providers on client’s needs and to reinforce/increase partnerships in relevant areas.
  • Participates in team meetings and trainings as assigned.
  • Other duties as assigned.

 

Qualifications

Experience:

  • MSW candidates must have at least 3 years of experience (at least 5 years of experience of candidate holds a BSW) working with vulnerable populations. 
  • Demonstrated ability providing trauma-informed assessment, crisis intervention, safety planning and other activities to support the safety and stabilization of a client’s placement in the community required.
  • Prior experience identifying community social services and other resources required.
  • Experience navigating complex systems (child welfare, immigration, court system, etc) required.
  • Prior clinical experience working with clients in acute crisis required.

 

Skills:

  • Fluency in verbal and written Spanish and English preferred.
  • Strong rapport and trust building skills.
  • High degree of emotional intelligence, ability to remain calm and use sound judgement in high stress situations.
  • Receptive to growth and learning.

 

Education/ Certifications/ Licensures:

 

  • Master’s degree in social work, psychology, sociology, or other relevant behavioral science in which direct clinical experience is a program requirement highly preferred.  A Bachelor’s degree in similar field with at least five years of clinical experience may be considered
  • Possess and maintain active clinical licensure. 
  • Certifications or training in Motivational Interviewing, Youth Mental Health First Aid, Trauma-Focused Cognitive Behavioral Therapy preferred.
